The Form Builder functionality enables you to provide tailor-made forms in your client catalogue which prompts clients to provide you with all the information you need upfront, reducing the time of liaising back and forth.


These forms can be utilised as creative brief form, detail specification forms, etc.


Alternatively, Form Builder can also be used to create Research Questionnaires within the E-procurement module.


Create a form:

  • Head to Settings> Form Builder.
  • Select Build New Form (bottom of the page beneath existing forms) to start creating the form.
  • Give your form a name in the 'Form name' field.
  • In the 'Form purpose' field, choose:
    • Catalogue or Job Cost: in order to create a form that can be used as a Quote Request.
    • Research: in order to create a form that can be used as a Research Questionnaire.
    • Organisation Data Slots: in order to create a form that can be used for Organisation Data Slots.
    • Ordering notes: in order to create a form that can be attached to Catalogue Items.
    • RFX Quote (Sourcing): in order to create a form that can be used as an RFX Quote questionnaire.
  • Add your questions by clicking on each question type:
    • Single-line text field - One line answer.
    • Multi-line text-field - Textbox allowing more detailed text, and spaced on different lines.
    • Drop-down list - Option to select one item from drop-down list.
    • Radio button group - Option to select one answer only, perfect for those "Yes/No" questions.
    • Checkbox group - Ability to select multiple options.
    • Date Selector - Option to specify a date, e.g. final proof deadline.
    • Text block - Ability to include text you'd like the client to see when responding to the form. Note this is read-only, and you can also choose to Include in response which will allow the text block to show in the order specifications.
    • Data slots - Enables a supplier to fill in/use a pre-populated answer from an organisation data slot in a Research Questionnaire.
  • To note:
    • For Data slots - there is a drop-down menu which enables you to pick which data slot you wish to include.
      • When a supplier fills in the form, if they have the value already populated in the data slot then it will automatically be pulled into the Research form as their response.
      • If they enter a new value for the data slot through the Research form, the value of the data slot is also updated in their profile.
    • For the Drop-down list, Checkbox, and Radio button – under ‘options’ press Add to fields to be picked from the drop-down list
    • Change order of questions by pressing and dragging the box with multiple squares next to it.
    • Help text – Set text under the field box.
    • Placeholder text – Shows in the field box, acts as a prompt.
    • Option to make fields required, so it cannot be submitted without information inputted.
  • Select the Preview tab to view how the form would look.
  • When happy press Create.
  • If you need to amend anything, make your changes and then press Update.
  • You can also copy the form over by pressing the grey button next to the forms.


If you would like to start using these bespoke forms, please contact our support team at Panacea Software so we can then attach the form to the relevant item in your Client Catalogue by creating a Cost Description. These forms can then be used as quote requests within the Client Interface, and will follow the standard workflow for Quote Requests.  NB: This is not relevant for creating Research Questionnaires if you have our E-procurement module.


It's important to note that suppliers will be need to be qualified against the newly created form in order to send suppliers quote requests.

  • Organisation > Goods and Services > Tick against the relevant form (Cost Description title).


Master Organisation Data Slot Form:


(Relevant only if Data Slots used) The data slot functionality is stored within one single Master Organisation Data Slot Form created. 

  • Here you can add commonly asked questions and requests for data which suppliers are often asked. Suppliers can then upload this information to the particular Data Slots, making it easily accessible. 
  • You can also add the RAG option, which enables Suppliers to be ranked as either Red, Amber or Green according to specific criterion. So, a specific question can be asked or specific criteria described and the Supplier's ranking in relation to these can be recorded.
